Complete the Application Form below to apply for an OFFICIAL legally binding Change of Name DEED POLL. 

After you submit your application online our legal team will draw up and issue your Deed Poll so when you receive its ready to be signed by you and your witness in the presence of a Commissioner for Oaths or a Solicitor.

When you receive the  Deed in the post, you must "enact" the Deed by signing it with your witness in the presence of a Commissioner for Oaths or a Solicitor who will also co-sign. 

You can then register the deed poll yourself at the Deed Poll Office which is recommended.

A Deed Poll is used to in Ireland change your name legally and can be applied for by anyone 18 years of age and over.

Parents can apply to change their child's name by Deed Poll and Adolescents (14-17 years of age) can apply with the written consent of their parents.

Note: Due to demand issue of a deed can take up to 6 weeks so these documents are not eligible for express issue.

Note: When completing the CURRENT name below - list it as it appears on the Birth Certificate of the Adult or Child/Adolescent-including all FORENAMES.

Note: After you change your name by Deed Poll your Birth Certificate does not change. When asked to prove your name by Government bodies/officials/financial institutions, you must present your Birth Certificate AND your Change of Name Deed Poll as proof of your entitlement to use a new name.

Note: The Irish Passport Office will only allow an adult/adolescents to change your name on your Passport when you can provide proof of using your new name for TWO YEARS and varying timespans apply for this to children.

IMPORTANT: Applications from Non-EU Passports Holders will need to submit to our office a Letter of Authorisation from the Irish Dept of Justice before we can issue your Change of Name Deed Poll.

Deed Poll Witness

  • A Deed Poll for Change of Name MUST be WITNESSED by a person you nominate to co-sign the Deed Poll with.
  • Your WITNESS must be over 18 years of age and read & speak English fluently.
  • You AND your WITNESS will need to SIGN the Deed Poll TOGETHER in the presence of a local Commissioner for Oaths /Peace Commissioner.
  • Without this step your Deed Poll is not complete legally.

Optional: High Court Registration

Do you wish to Register Your Deed Poll With the High Court?
When we issue your Deed Poll you can submit it to the High Court for it to be listed on the Register of Deed Polls registered in the Court Records.

You can submit the Deed Poll without a Solicitor to the Deed Poll Office in Dublin.

You are NOT legally obliged to do this but there are some advantages to High Court registration.

  • You have lodged your change of name publicly with the State and it appears on the Register of Deed Polls which while not required legally supports your claim to a new name.
  • The High Court will issue a copy to your Deed Poll if you lose it in the future.
  • An increasing number of government bodies like the National Driver's Licence Service and the PPS Number Section of the Dept of Social Protection will only accept a Deed Poll if its been lodged with The High Court
  • Do note the Irish Passport Office will only accept your Deed Poll if it is registered with the High Court AND you have proof of using your new name for a period set by their office.
